401K Plan out of Blackout!
Effective immediately all eligible 401(k) plan participants will be able to log into The 401k Company website at
www.401kaccess.com/LifePoint
or call their Customer Support or Voice Response Unit at 1-800-777-401k (1-800-777-4015). |

All employees will need an Account Number (SSN) or Web ID and web password. Your initial web password will be the last four digits of your Social Security number. You will be prompted to change it at after the initial long on. |

As previously communicated by corporate, if you are a prior participant in the LifePoint Plan, your funds were mapped into similar funds and you will now be able to change your investment elections and move into the new funds. (Due to Black-Out, the exception will be employees in the Brentwood office who cannot move in or out of the Stock Fund until Mid-February.)
As an eligible participant, you can now elect a deferral percentage which will be effective within the next two pay periods, depending on the date of the change and timing of files from The 401k Company to our Payroll system.
Because of the conversion timing, you will want to note that the Stock Fund will continue to be unitized (as with the prior recordkeeper) until mid-February. At that time it will be converted to share accounting, which means as a participant you will be able to see the number of LifePoint shares you hold at any time without requiring an “equilivant share” calculation.
The 401k Company was able to fully reconcile prior plan funds with your account balance! What that means is that your first statement from The 401k Company will reflect the amount on your 12/31/07 statement from Your Benefits Resources when received.
HR is in process of scheduling 401K Presentations. The date of this presentation is tentatively scheduled on Friday, February 29, 2008. Once the meeting times have been finalized we will inform all employees by our hospital Communique. We will also post flyers throughout the hospital. I hope each of you will be able to attend a presentation, but know you can always ask your HR Department or the Corporate HR Department if you have questions.
Recently each of you should have received a Conversion Brochure at your home address; however, I am attaching a pdf version for your convenience.

2008 Benefits Information - New ID Cards |
New ID card update:
Medical ID Cards - BCBST will be mailing new ID cards the week of December 12, 2007 to each participant. The new card is only for medical coverage beginning 1/1/08.
Prescription ID Cards - Caremark will be mailing new IDs to each participant the week of December 16, 2007. The new card is only for prescription coverage beginning 1/1/08.
Dental ID cards - Delta Dental ID cards for new participants will be mailed from Delta within the next two weeks.
REMINDER
Please begin using the new medical and new prescription ID cards starting 1/1/08. If a prescription is filed under the old ID card on/after 1/1/08, it will be denied.
To prevent denials, participants must tell their pharmacy about the new Caremark prescription ID card and use the new card starting 1/1/08.
